The Moose I.odge receives a Food Service license from the Board of Aealth. The cigarette machine is not to be relocated behind the bar or in the kitchen area. Mr. Sullivan stated he would remove the cigarette machine within two weeks. Chauman Fettes thanked Mr. Sullivan for his cooperation. I� Domino's - Hearinas Gregg Foa�Manager of Domino's Pizza, stated he was before the Board of Health last year to discuss a deviation from the water usage requirement noted on his Food Service license. The Director of Health stated the septic system was designed for retail use, however, after past discussions with the Engineer, Domind s was allowed into the unit with a water use restriction of 105 g.p.d. The manager is to maintain a monthly water usage log and submit a yearly report to the Health Depaztmern. A review of the 1998 water records revealed the unit was using approximately 130 g.p.d. William Lieberman, P.E., submitted a letter dated February 15, 1999 to the Boazd ofHealth. The letter states the water usage is consistent with the previous two years and that the contribution of Domino's Pizza is small. There is a lazge under utilized capacity and the septic system is perfornring to satisfactory condition. The Director of Health recommended the Boazd revise the water restriction to 175 g.p.d. which would be within the septic system capacity, as reported by William Lieberman, with the remaining restrictions on the license to be maintained. Domino's Pizza would still be required to maintain water records for submittal to the Board of Health. The Boazd agreed to accept the Director of Health's recommendation. The Director of Health informed the Boazd that during past food service inspections by Health Inspector,Kelda Welsh, the establishment was maintained in a very clean condition. y, Other Business A. The Boazd discussed the letter dated February 18, 1999 from Kelda Welsh to Union Station. Kelda Welsh stated they are going to hold an administrative hearing with Mr. Faye regarding maintenance and cleaning issues within the restaurant. Keida Welsh stated that the violarions on the food service inspection reports had been repet»tive violations throughout the past year. Page 3 of 4
