ALL GRAVITY SEWER PIPE SHALL BE POLYVINYL CHLORIDE PIPE (P.V.C.), S.D.R. 35 AND SHALL CONFORM WITH ASTM -D3034 UNLESS NOTED OTHERWISE. 14. THE CONTRACTOR SHALL GIVE THE GAS COMPANY ADVANCE NOTICE OF WHEN THE GAS LINE CAN BE INSTALLED. THE CONTRACTOR IS RESPONSIBLE FOR ALL EXCAVATION AND BACKFILL AND COMPACTION FOR THE GAS LINE. 16. EXACT LOCATION OF UTILITY STUBS FOR BUILDING CONNECTIONS SHALL BE VERIFIED WITH THE BUILDING DRAWINGS. SERVICE STUBS TO THE BUILDING SHALL BE INSTALLED TO A POINT 5 FEET FROM THE BUILDING WALL UNLESS OTHERWISE NOTED OR DETAILED, SHALL BE PROVIDED WITH TEMPORARY PLUGS AT THE END AND THE INSTALLED LOCATIONS SHALL BE MARKED AND IDENTIFIED WITH WITNESS STAKES AND PROTECTED. 17. DIMENSIONS SHOWN ARE TO CENTERUNE OF PIPE OR FIRING. 18. THE FINAL PRIMARY ELECTRICAL DISTRIBUTION SYSTEM WILL BE AS DESIGNED AND SPECIFIED BY THE UTILITY AND IS SHOWN GRAPHICALLY FOR COORDINATION PURPOSES. ALL OVERHEAD POLES AND WIRES TO BE INSTALLED BY THE UTILITY CO. 19. ALL EROSION AND SEDIMENTATION CONTROL MEASURES SHALL BE CONSTRUCTED IN ACCORDANCE WITH DEP'S STORMWATER TECHNICAL HANDBOOK (MARCH 1997), AND NRCS'S MASSACHUSETTS EROSION AND SEDIMENT CONTROL GUIDELINES FOR URBAN & SUBURBAN AREAS (MARCH 1997) AND ALL LOCAL MUNICIPAL REGULATIONS, AND THE STORMWATER POLLUTION PREVENTION PLAN (SWPPP), ALL LOCAL MUNICIPAL REGULATIONS, AND THE STORMWATER POLLUTION PREVENTION PLAN (SWPP). 2. EROSION AND SEDIMENTATION CONTROL MEASURES SHALL BE IN PUKE PRIOR TO THE COMMENCEMENT OF ANY SITE WORK OR EARTHWORK OPERATIONS, SHALL BE MAINTAINED DURING CONSTRUCTION, AND SHALL REMAIN IN PLACE UNTIL ALL SITE WORK IS COMPLETE AND GROUND COVER IS ESTABLISHED,. 3. THE CONTRACTOR SHALL NOTIFY THE TOWN'S PLANNING STAFF AND CONSERVATION OFFICER AT LEAST 48 HOURS PRIOR TO COMMENCEMENT OF ANY SITEWORK. 4. THE CONTRACTOR SHALL AT HIS EXPENSE SURVEY AND MARK OUT IN THE FIELD THE LIMITS OF CLEARING (I.E. HAYBALE/SILTFENCE LINE) AND RE -FLAG (WHERE NECESSARY) THE WETLAND BOUNDARY FOR APPROVAL BY THE TOWN PRIOR TO INITIATION OF CLEARING. 5. THE LIMIT OF WORK LINE FOR THE AREA TO BE CLEARED AND GRUBBED SHALL BE THE SAME AS THE LIMIT OF WORK LINE NECESSARY FOR GRADING PURPOSES, (IE., THE GRADING LIMITS AROUND THE PERIMETER OF THE PROJECT AREA). THIS LINE IS THE HAY13ALE-WITH SILT FENCE LINE OR AS NOTED. THE SILT FENCE LINE IS THE LIMIT OF WORK WHERE NO HAYBALES ARE SHOWN. 6. PRIOR TO INITIATION OF CONSTRUCTION ACTIVITIES AT THE SITE, THE CONTRACTOR SHALL ENGAGE AN INDIVIDUAL WITH SPECIFIC PROFESSIONAL TRAINING AND EXPERTISE IN EROSION AND SEDIMENT CONTROL. THE EROSION CONTROL MONITOR SHALL PREPARE REPORTS WHICH SHALL BE KEPT ON SITE AT ALL TIMES AND SHALL BE SHOWN TO LOCAL, STATE, AND FEDERAL AGENTS UPON REQUEST. THE REPORTS SHALL INDICATE THE STATUS OF THE EROSION CONTROLS AND ANY MAINTENANCE REQUIRED AND PERFORMED. THE REPORTS SHALL CONFORM TO THE REQUIREMENTS OF THE EPA'S NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES) PERMIT AND THE STORMWATER POLLUTION PREVENTION PLAN (SWPP). 7. STOCKPILES SHALL BE SURROUNDED ON THEIR PERIMETERS WITH STAKED HAYBALES AND/OR SILTATION FENCES TO PREVENT AND/OR CONTROL SILTATION AND EROSION. THE LOCATION OF THE STOCKPILE AREAS MAY BE MODIFIED AS APPROVED BY THE ON-SITE EROSION CONTROL MONITOR, BUT SHALL BE LOCATED OUTSIDE THE 100' WETLAND BUFFER ZONE. 8. TOPS OF STOCKPILES SHALL BE COVERED IN SUCH A MANNER THAT STORMWATER DOES NOT INFILTRATE THE MATERIALS AND THEREBY. RENDER THE SAME UNSUITABLE FOR FILL USE. 9. ALL DISTURBED OR EXPOSED AREAS SUBJECT TO EROSION SHALL BE STABILIZED WITH MULCH OR SEEDED FOR - TEMPORARY VEGETATIVE COVER WHERE CONSTRUCTION ACTIVITIES HAVE PERMANENTLY CEASED OR HAVE TEMPORARILY BEEN SUSPENDED FOR MORE THAN FOURTEEN DAYS, OR WHEN FINAL GRADES ARE REACHED IN ANY PORTION OF THE SITE, STABILIZATION PRACTICES SHALL BE IMPLEMENTED WITHIN THREE DAYS. AREAS WHICH REMAIN DISTURBED BUT INACTIVE FOR AT LEAST THIRTY DAYS SHALL RECEIVE TEMPORARY SEEDING. IN ALL CASES, STABILIZATION MEASURES SHALL BE IMPLEMENTED AS SOON AS POSSIBLE AND IN ACCORDANCE WITH THE EROSION AND SEDIMENT CONTROL GUIDELINES. - 10. EARTHWORK ACTIVITY ON THE SITE SHALL BE DONE IN A MANNER SUCH THAT RUNOFF IS DIRECTED TO TEMPORARY DRAINAGE SWALES AND SEDIMENTATION BASINS PRIOR TO DISCHARGE FROM THE SITE. 11. TEMPORARY DRAINAGE SWALES, STONE CHECK .DAMS AND SEDIMENTATION TRAPS SHALL BE UTILIZED AS NEEDED AND THEY SHALL BE RELOCATED AS REQUIRED AS CONSTRUCTION PROGRESSES. 12. CULVERT/PIPE INLETS AND OUTFALLS SHALL BE PROTECTED BY HAYBALE FILTERS UNTIL DISTURBED AREAS ARE PERMANENTLY STABILIZED. 13. HAYBALE DIKES SHALL BE CONSTRUCTED AT ALL EXISTING & PROPOSED CATCH BASINS LOCATED IN FILL AREAS & SUBJECT TO STORMWATER RUN-OFF FROM PROPOSED FILL AREAS DURING CONSTRUCTION, OR AS DIRECTED BY THE OWNER/ENGINEER. NO SEDIMENTS SHALL ENTER THE ON-SITE OR OFF-SITE DRAINAGE SYSTEMS AT ANY. TIME. 14. SILTATION FILTER BAGS SHALL BE INSTALLED INSIDE CATCH BASINS IN HIGH TRAFFIC AREAS WHERE HAYBALES WILL INTERFERE WITH TRAFFIC AND OTHER PAVED AREAS WHERE NOTED OR AS NEEDED. HAYSALES SHOULD NOT BE INSTALLED ON PAVED SURFACES. 15. ALL EROSION CONTROL MEASURES SHALL BE ROUTINELY INSPECTED, CLEANED AND REPAIRED OR REPLACED AS NECESSARY THROUGHOUT ALL PHASES OF CONSTRUCTION. IN ADDITION, INSPECTION SHALL TAKE PLACE AT LEAST ONCE EVERY (7) CALENDAR DAYS AND WITHIN 24 HOURS FOLLOWING ARAINFALL EVENT EXCEEDING 0.5" OF PRECIPITATION. 16. THE SUBSURFACE DETENTION BASIN SHALL NOT, AT ANY POINT DURING CONSTRUCTION, BE USED AS SEDIMENTATION BASINS. ANY DEWATERING REQUIRED IN CONSTRUCTING THE DETENTION AREAS OR FOR OTHER FEATURES ON THE SITE SHALL DISCHARGE INTO A TEMPORARY STILLING BASIN OUTSIDE OF A 100 FOOT BUFFER ZONE FROM WETLANDS. 18. THE LOCATION OF HAYBALE CHECK DAMS SHALL BE FIELD VERIFIED AND ADJUSTED AS NECESSARY DURING SITE PREPARATION OPERATIONS BY THE CONTRACTOR. 19. ALL PROPOSED SLOPES STEEPER THAN 3:1 SHALL BE STABILIZED WITH JUTE MESH AND PROTECTED FROM EROSION. 20. THE CONTRACTOR SHALL KEEP ON SITE AT ALL TIMES ADDITIONAL HAYBALES AND EXTRA SILTATION FENCING FOR INSTALLATION AT THE DIRECTION OF THE ENGINEER OR THE TOWN TO MITIGATE ANY EMERGENCY CONDITION. 21. THE AREA OR AREAS OF ENTRANCE AND EXIT TO AND FROM THE SITE SHALL BE CLEARED OF ALL VEGETATION, ROOTS AND OTHER OBJECTIONABLE MATERIAL AS DETERMINED BY THE ENGINEER OR OWNER'S REPRESENTATIVE. 22. THE AREA OR AREAS OF ENTRANCE AND EXIT TO AND FROM THE SITE SHALL BE MAINTAINED IN A CONDITION WHICH WILL PREVENT TRACKING OR FLOWING OF SEDIMENT ONTO PUBLIC RIGHT-OF-WAY.. ALL SEDIMENT SPILLED, DROPPED, WASHED OR TRACKED ONTO PUBUC RIGHT-OF-WAY MUST BE REMOVED IMMEDIATELY. 23. THE CONTRACTOR SHALL BE AWARE THAT THIS SITE IS SUSCEPTIBLE TO SOIL EROSION AND SENSITIVE TO ITS CONSEQUENCES. IT SHOULD BE NOTED THAT THE EROSION CONTROL MEASURES AS SHOWN ON THE DRAWINGS DEPICT THE MINIMUM REQUIRED CONTROL AND ARE REPRESENTATIVE OF A SINGLE STAGE OF CONSTRUCTION FOR EACH PHASE.
