Decision 4396 - E - Utility & Lighting Plan
UTILITY NOTES: 1. THE CONTRACTOR SHALL VERIFY THE LOCATION AND ELEVATION OF ALL EXISTING UTILITIES AND STRUCTURES PRIOR TO THE START OF CONSTRUCTION. ANY NOTED DISCREPANCIES SHALL BE REPORTED TO THE ENGINEER. 2. THE CONTRACTOR SHALL BE RESPONSIBLE FOR MAINTAINING ADEQUATE RECORDS OF THE LOCATION AND ELEVATION OF ALL WORK INSTALLED. THE CONTRACTOR SHALL PROVIDE THIS INFORMATION TO THE ENGINEER UPON REQUEST, FOR USE IN PREPARING AS—BUILTS. 3. UNLESS OTHERWISE NOTED, ALL EXISTING UTILITIES THAT SERVE EXISTING USES/BUILDINGS, INCLUDING BUT NOT LIMITED TO, DRAIN, GAS, TELEPHONE, CABLE AND ELECTRIC SHALL BE PROTECTED. THESE UTILITIES AND OTHERS THAT SERVE ADJACENT PROPERTIES SHALL REMAIN IN SERVICE THROUGHOUT CONSTRUCTION. 4. ELECTRIC, TELEPHONE, CABLE, AND GAS UTILITY SIZES, AND APPURTENANCES SHALL BE INSTALLED IN ACCORDANCE WITH REQUIREMENTS OF THE APPROPRIATE UTILITY COMPANIES. 5. THIS PLAN HAS BEEN PREPARED AS A SCHEMATIC REPRESENTATION OF THE PROPOSED ELECTRIC, TELEPHONE, GAS, AND WATER UTILITY SERVICES. IT IS NOT THE INTENTION OF THIS DRAWING THAT EVERY DETAIL, DEVICE, ITEM OR PIECE OF EQUIPMENT BE SHOWN. FINAL DESIGN AND APPROVAL MUST BE OBTAINED FROM THE RESPECTIVE UTILITY COMPANIES FOR LOCATION OF TRANSFORMERS, CONDUITS, GAS METERS AND SERVICE CONNECTIONS. 6. AT THE COMPLETION OF OPERATIONS, THE CONTRACTOR SHALL BE RESPONSIBLE FOR CLEANING ALL NEW AND EXISTING DRAINAGE STRUCTURES OF SAND, SOIL OR OTHER DEBRIS. 7. INSTALLATION AND TESTING OF THE WATER MAINS, SERVICE CONNECTIONS, GATE VALVES/BOXES, HYDRANTS, AND FITTINGS SHALL CONFORM TO THE TOWN OF YARMOUTH REQUIREMENTS. 8. SIZE OF GAS LINE SHALL BE DETERMINED BY THE LOCAL GAS PROVIDER. SEWAGE DISPOSAL SYSTEM NOTES: 1. THIS PLAN IN INTENDED FOR THE DESIGN AND CONSTRUCTION OF THE SEWAGE DISPOSAL FACILITY. 2. ALL CONSTRUCTION METHODS AND MATERIALS SHALL CONFORM TO 310 CMR 15.000 AND YARMOUTH BOARD OF HEALTH REGULATIONS. 3. THERE ARE NO KNOWN OR PROPOSED PRIVATE WELLS LOCATED WITHIN 150 FEET OF THE PROPOSED LEACHING FACILITY. 4. EXISTING UTILITIES WHERE SHOWN ON THE PLANS ARE APPROXIMATE. THE ENGINEER DOES NOT GUARANTEE THEIR ACCURACY OR THAT ALL SUBSURFACE STRUCTURES ARE SHOWN. CONTRACTOR SHALL VERIFY THE SIZE, LOCATION AND ELEVATION OF INVERTS OF UTILITIES AND STRUCTURES, WITHIN THE LIMIT OF WORK, PRIOR TO THE START OF CONSTRUCTION. IF ANY DISCREPANCIES ARE DISCOVERED OR FIELD CHANGES REQUIRED, THE CONTRACTOR SHALL NOTIFY THE ENGINEER IMMEDIATELY. 5. THE CONTRACTOR SHALL BE RESPONSIBLE FOR PROPERLY COORDINATING THE PROPOSED CONSTRUCTION ACTIVITIES WITH DIGSAFE AND THE APPLICABLE UTILITY COMPANIES, AND SHALL COMPLETE THE PROPOSED WORK WITHOUT ANY INTERUPTIONS IN SERVICE. 6. CONTRACTOR IS REQUIRED TO NOTIFY DIGSAFE, PER MASS. STATUTE CHAPTER 82, SECTICN 40 (1-888-344-7233) A MINIMUM OF 72 HOURS PRIOR TO THE START OF CONSTRUCTION. 7.
