GENERAL NOTES:
I. THIS PLAN IS FOR THE DESIGN AND CONSTRUCTION
OF THE SEWAGE DISPOSAL SYSTEM AND PERMITTING
PURPOSES ONLY.
2. VERTICAL DATUM IS NGVD. FOR BENCH MARKS
SET. SEE SITE PLAN.
J. ALL CONSTRUCTION METHODS AND MATERIALS AND
MAINTENANCE OF THE SEPTIC SYSTEM SHALL
CONFORM TO MASS. D.E.P. TITLE 5 AND LOCAL
BOARD OF HEALTH REGULATIONS.
4. ALL SEPTIC SYSTEM COMPONENTS LOCATED UNDER
AREAS SUBJECT TO VEHICULAR TRAFFIC OR GREATER
THAN 3' IN DEPTH SHALL BE CAPABLE OF WITH-
STANDING H-20 WHEEL LOADS.
5. ALL SEWER PIPE SHALL BE SCHEDULE 40 OR
APPROVED EOUAL. .
6. SEPTIC TANK. PUMP CHAMBER AND D -BOX SHALL BE
REINFORCED PRECAST CONCRETE. WATERTIGHT AND
WATERPROOF. D -BOX SHALL BE WATER TESTED TO
CHECK FOR LEVEL WHEN THERE IS MORE THAN ONE
OUTLET.
7. BEFORE CONSTRUCTION CALL 'DIG -SAFE".
1 -888 -DIG -SAFE AND THE LOCAL WATER DEPT.
FOR LOCATION OF UNDERGROUND UTILITIES.
8. SEPTIC SYSTEM INSTALLER SHALL NOTIFY THE
DESIGN ENGINEER TWO DAYS PRIOR TO CONSTRUCTION
OF THE SYSTEM TO ALLOW FOR SCHEDULING OF THE
CONSTRUCTION INSPECTIONS. ENGINEER TO INSPECT:
1. OVERDIG AND STAKE WALL.
2. FOOTING AND REBAf BEFORE POURING WALLS.
J. START UP OF PUMPS PRIOR TO INSTALLATION OF
6" HALFPIPE.
4. FINAL INSPECTION.
9. EXISTING SEPTIC SYSTEM BE PUMPED DRY. REMOVED
AND/OR BACKFILLED.
10. ALL UNSUITABLE MATERIAL (A 6 B HORIZONS)
ENCOUNTERED BELOW THE INVERT OF THE LEACHING
FACILITY TO BE REMOVED FOR A DISTANCE OF 5'
AROUND AND REPLACED WITH SAND IN ACCORDANCE
WITH TITLE 5.
11. NO DETERMINATION HAS BEEN MADE AS TO
COMPLIANCE WITH DEED RESTRICTIONS OR ZONING
REGULATIONS. IT SHALL REMAIN THE CLIENTS
RESPONSIBILITY TO OBTAIN ALL PERMITS. SPECIAL
PERMITS, VARIANCES ETC. FOR THIS PROJECT.
12. IT SHALL REMAIN THE CLIENT'S RESPONSIBILITY
TO HAVE THE PROPOSED BUILDING FOUNDATION
DESIGNED TO ACCOUNT FOR THE EXISTING GRADE
AND SOIL CONDITIONS AT THE LOCATION OF THE
PROPOSED BUILDING.

10' IS REOUIRED BETWEEN THE SAS AND THE PROPERTY LINE. 3' 15
PROVIDED TO THE ABUTTER. A 7- VARIANCE IS REOUESTEO.
PUMP SYSTEM NOTES:
1. PWP TO BE MYERS RESIDENTIAL SEWAGE PUMP MODEL SRM4 4' PVC INLET
OR EQUAL.
2. THE PUMP SHALL START AND STOP AT THE ELEVATIONS SHOWN.
J. THE PWP SHALL BE INSTALLED IN STRICT CONFORMANCE WITH
THE MANUFACTURER'S SPECIFICATIONS AND .TITLE V REGULATIONS.
PUMP DISCHARGE SHALL BE 2 INCHES. PUMP SHOULD BE ABLE TO
BE DISCONNECTED AND LIFTED OUT OF THE PWP CHAMBER WITHOUT
HAVING TO ENTER THE PUMP CHAMBER.
4. THE ALARM SHALL START AT THE ELEVATION SHOWN AND BE
POWERED BY A CI RCUI T SEPARATE FROM .THE PWP POWER-
S. AN ELECTRICAL PERMIT MUST BE OBTAINED FOR THIS INSTALLATION.
