to conduct soil evaluations and that the above analysis has been performed by me consistent with the
requirements of 310 CMR 15.017. 1 further certify that I have passed the Soil Eval. Exam in October, 1999.
GENERAL NOTES:
1. ALL CHANGES TO THIS PLAN MUST BE APPROVED BY THE LOCAL BOARD OF HEALTH AND THE DESIGN ENGINEER.
