On June 17, 2011 a final inspection was done and measurements were taken for an"as-built"plan,per
your request. The septic system was installed using the approVed plan, dated October 20, 2010,with the
installer using different components and placing the system in a different location. The following
notations also reflect my inspection:
1. The SAS components were changed to ADS 16 inch High Capacity Bio Diffusers, instead of the
High Capacity Infiltrators shown on approved plan.
2. The entire SAS and D-Box have been moved,at minimum, 33 feet westerly.
3. The entire SAS and D-Box are approximately 2 feet higher than designed.
4. The Septic Tank is approximately 1 foot lower than designed.
5. The Top of Foundation is 1.4 feet lower than designed.
6. The gra.ding over the SAS will be a concern.
7. The pipe from the Septic Tank to the house i�not connected to the interior piping(as of the
inspection date).
8. All the interior waste lines are on the front wa11. It is recommended that the exterior waste line run
from the front of the house to the side inlet of the Septic Tank.
' 9. The HomeownerBuilder has made a walkout basement in the rear northeast comer. Grading will
be problematic as well as drainage.
10. With the house rotated,the driveway will have grading issues.
11. The system needs to be carefully backfilled and graded.
The As-Built card will be turned in to your office by either Adam Riker(holder of the Pernut� or
Kerry Sullivan,Dig-It Construction,who installed the system.
My As-Built Plan with elevations is atta.ched to this Certification Letter. If you ha.ve any questions or
