We are in receipt of a letter from James Smith dated January 19, 2016 stating that he is aware
that an H-20 D-box was specified on the plan, and that an H-10 D-box was installed, and that he
understands the change was made.
The H-10 D-box installed satisfies Title 5 requirements for this system,therefore we approve the
change to an H-10 D-box.

recommended H-20.
I understand that the newly installed distribution box is not in a traffic area and is not required to be H-
20 rated.
I understand that this installed distribution box does not have any affect on the septic system operation.
