1 compieted the final inspection for the recently upgraded sewage disposal system at 5 Par Three
Drive, South Yarmouth, MA. �I
The upgraded system consisted of a new 1,500 gallon Septic tan, an H-20 DB-6 Distnbution Box,
and Twenty(20)ADS Arc 36HC(3616BD2)Leach Chambers in a bed configuration.
The subsurface sewage disposal system installed at the above referenced property has been
constructed in substantial compliance with 310 CMR 15.000, all local requirements of the
Yarmouth Board of Health,and the approved design plans as filed with your office.
