I have reviewed the plan for 44 Phyllis Drive and everything is all set on the plan. Prior to issuing the permit for
installation to Mike Bosetti, I will need a completed 2 Bedroom Deed Restriction application from you. The
deed restriction will then need to be processed at the Registry and a copy returned to our office prior to
releasing the Certificate of Compliance after the system is installed.
