Dear Ms. Rotella-Garzon,
I have received the letters regarding the change from Hess Corp. to Hess Retail Operation, LLC, in relation to the retail food service and tobacco sales licensing of the three Hess Express
establishments located in the Town of Yarmouth. I have also consulted with the Health Director Bruce Murphy regarding the situation.
There must have been a misunderstanding by the person with whom you spoke about the change. Unfortunately I must inform you that letters of notification are not sufficient. If the corporate
entity had remained the same, but the establishment name had changed, then there only would be a name change fee of $15.00. However, in this case, since the corporate entity has changed,
as well as the FEIN number, new licensing is required. Licenses are not transferable from one corporate entity to another.
The fee for each retail food service license is $80.00 and for each tobacco sales license is $95.00. I have attached the license application and the workers compensation affidavit form
for your convenience.
