The Yarmouth Board of Health is in receipt of your application reqvesting a variance from the
State Environmental Code's Title 5 and/or Town of Yarmouth Amendments to Title 5. Plans
� of the septic system are on file, for review, at the Health Department Office.

DESCRIPTION OF REQUESTED VARIANCE(S):
1 . Primary leaching system to be located 164 ' from well. No variance
required from the 150' required seperating distance.
2. Reserve leaching system to be located 136 ' from well. Variance of
14 ' .
Septic system to be installed per Engineered Plans. (Available at
� Health Office)
� Sewage system plans have been revised to show final grading to contain
surface water run off per previous Board of Health meeting of 2/3/86 .
The sewage system has been relocated to be further away from the well
then previously requested.
Please be advised that the Yarmouth Board of Health will conduct a Hearing for the above
requested variance(s) on: March 3 , 19 8 6 at 8 :15 P•m• �
the Lower Meeting Room of the Town Office Bldg., Route 28, South Yarmouth, MA.
NOTE: The Engineer, whose stamp appears on the prepared plan(s), must be present at the
Board of Health Meeting.
