APPLICATION for REVIEW
 TO: Bruce Murphy
 RE: Duplex 8a-8b & 12a-12b Date: 02/4/2016
Rosemary Ln. West Yarmouth
Greeting Bruce,
I, Willy Planinshek( Lic. CSL and General Contractor) represent Nancy
Johnson in this matter and request your review of 2 - Title 5 Official
Inspection Reports submitted by a TK Septic Inspections on or about Jan 6
2016.
Upon receipt of the said reports in early December, I visited Rosemary Ln. I
found no evidence of inspection digging: the leaching fields, "D" boxes nor
solid tanks. 'There was a small area of ponding due to the parking of cars and
recent rain - no odor of sewage. A couple of tenants were on site and I
inquired if anyone had been seen digging or had asked if there were any
outside or inside sewage problems, both negative.
I informed my client Nancy as to my observations and asked if I could get a
second opinion, she agreed.
The 1/28/2016 reports indicate a Conditional Pass, I was told due to the "D"
boxes.
I ask the Health Dept. to now rescind requirements outlined in letter dated
January 20, 2016 and to impose the Conditional Pass improvements with any
subsequent inspections the Health Dept. deems appropriate.
Regards,
Willy Planinshek
