For the duration of this contract beginning July 25,2015 and concluding July 25,2016 ,
the Service Provider shall operate and maintain the SeptiTech Wastewater Treatment system in ',
accordance with conditions imposed by the Massachusetts Department of Environmental ',
Protection and requirements set forth by the local Board of Health. The inspector will perform ',
visual inspection of effluent wastewater and perform the following field test Annually.
1. Visual examination of effluent for color,turbidity and effluent solids.
2.Test effluent PH to determine the wastewater reads between 6 and 9 standard units.
3. Dissolved oxygen to be 2mg/L or more to ensure operation of the system.
4.Turbidity, less than or equal to 40 NTU.
If field tests show a poor quality effluent,the Department of Environmental Protection requires
a laboratory analysis of effluent of the following:
A. Carbonaceous Biological Oxygen Demand {CBOD)
B.Total Suspended Solids(TSS)
C. PH and Temperature
Costs for this collection and laboratory testing if required will be the responsibility of the '
homeowner at a cost of$200.00 per sample. -----

If a system fails to meet required test parameters homeowner agrees to authorize and pay for
any additionai repairs or re-testing costs as needed.
Service Provider shall submit all inspection reports and results to the MA DEP and local Board
of Health and notify the homeowner of any necessary repairs. '

Owner agrees to immediately notify Rosano Davis Sanitary Pumping, Inc. in the event of an �
alarm, system failure or malfunction. Rosano Davis Sanitary Pumping, Inc. agrees to respond to
this notification within 48 hours and to take immediate corrective action to remedy the
situation. G
Any service calls required or requested by the homeowner beyond normal operation and '
maintenance shall be billed at a rate of$75/hr. per person plus mileage and travel time,with a ;
one hour minimum charge for all service calls.
Owner agrees that the Service Provider shall have access to the treatment unit and control
panel. Any access ports/covers buried under grass or mulch will automatically void any
manufacturer's warranty of the system.
Fees: Routine operation & maintenance:�135.00 per inspection. �
Barnstable County Database User' Fee: $60.00 Annually �k

Full pavment for this 1-vear contract is due upon si�nin�.
This agreement shall be assignable in the event the real estate is sold. The new owner shall
enter into agreement with the service provider for the operation and maintenance of the on-
site wastewater treatment system. The new owner shall assume all client obligations for the
remainder of this contract upon passing of papers on the property.
