Holyoak 7 Stone Avenue West Yarmouth, MA 02673 � i , RE: Denial for Monitoring Reduction for Innovative/Alternative Technolog,y 7 Stone Avenue,Yarmouth,MA—MicroFAST�0.5 System,DEP#0204842 Dear Mr. Holyoak: The Yarmouth Health Department is in receipt of your request to reduce the monitoring requirements allowed by the MA Title 5 Regulations, Innovative/Alternative Technology Approval Letter for your septic treatment system at the above address. Upon a review of your file, our records indicate that your system has not been in compliance with the contract and monitoring requirements, as required by the Yarmouth Board of Health Variance Approval Letter dated January 25, 2011. At a minimum,the MicroFAST unit was to be monitored and sampled twice per year, and tested for BOD, TSS,pH, and Tota1 Nitrogen for a minimum of two years at which time the applicant could request a reduction in testing from the Yarmouth Health ' Department. The Sample History for your property indicates only three sampling events have occurred since 2012. This Departxnerit hereby denies your request to reduce the maintenance and monitoring requirements of your MicroFAST system. You are hereby required to continue to maintain and test your system via a certified operator every six(6)months and any time there is an alarm � event, as specified in the Yarmouth Board of Health Approval Letter of January 25, 2011. This � denial will be effective for a minimum of two (2)yeazs from the date of this notice. Upon completion of your two year sampling regimen, you may apply for a reduction in the sampling and rnonitoring requirements with the Yarmouth Health Department.
