Dear Ms. Matthews:
Please find enelosed the Two Bedraom Deed Restrictian for the above address which is required
as part of the septic system approval process. T'he Deed Restriction must be signed and dated by
the owner(s) or legal designee in front of a notary. T'here are several notaries availa.ble at
Yarmouth Town Ha.11, if needed. Once the restriction is signed and notarized,please make a copy
and have both the Qriginal restrietian and the capy reevrded at the Barnstable County R'.�gistry af
Deeds, Route 6A, Barnstable. The copy of the recorded Restriction must then be returned to the
Health Department as proof of the recording.
