The Yarmouth Board of Health has received your application for a variance from the
State Environmental Code: Title 5 and/or from the Town of Yarmouth Amendments to
Title 5:
DESCRIPTION OF VARIANCE(S) REQUESTED:
For the Board of Health to grant a sig(6) month egtension of the Subsurface Sewage
Disposal Construction Permit.
The Subsurface Sewage Disposal System has been installed according to the Bngineered
Plans; however, the system is not in operation at the present time, as required by Section
2.4 of the Town Amendments to Title 5, which states that the system must be in opera-
tion within sis (6) months. At this time, only the foundation has been poured.

Please be advised that the Board of Health will conduct a hearing for the above re-
quested variance(s) on: �TOBER 15, 1984 at 7:30 p.m. p.m.
in the Lower Meeting Room of the Town Office Building, Route 28, South Yarmouth, MA.
