In reviewing our files for FY-97, it has come to our attention that your business has not submitted
a 1997 Permit Application, a violation of State Code. Enclosed, please find a Health Department
Application and Worker Compensation Affidavit. Please provide all necessary informatioq sign
the documents, and retum them to my attention with a check payable to the Town of Yarmouth.
Failure to comply with the above instructions will result in an administrative hearing to discuss the
future of your business and permits
