1. System is deliyned in accordance with "Commonweallh of Mass, Dept
of Environmental Pmtection, 310 C,ifR 1.5.00; the State Environmental Code,
Title 5 Standard Requirements for the Sitiog Construction, Inspection,
Upgrade and Expansion o� On Sewage Treatment and Disposal Systems"
and the Town of YARMOUTH Regulations.
2 No wells or water supplies are known toy�,7' st within feet of the
proposed leaching system. All wells krwwF4?4vxist within fl. of the
system are shown.
3 Prior to backfilling completed system, notify the Fngineer and the
Board of Health for inspection. Provide 94 hours (min.) notice.
V. Contractor shall be responsible for verifying location of all underground
utilities prior to excavation.
S Any changes to this plan must be approved by the Board of HFalth.
6. Area is served by town water.
