On the recent annual inspection of the property known as Anthony's Cummaquid Inn,
Yarmouth Port MA,there were some serious violations.
1. Cooking hood system needed cleaning and inspection.
2. Fire egtinguishers ezpired.
3. Sprinkler system inspection ezpired.
Note: I gave you the choice of removing the two twin beds on the 2°d floor or installing
smoke and carbon monoxide detectors to the ezisting system.
Our property file for Anthony's Cummaquid Inn indicates,from previous fire department
inspections,that in 2005,2006,2007,2008,2009,& 2010 the same types of violations
occurred over and over again.
The most disturbing discovery this year was the sign off on the sprinkler system tag
attached to the sprinkler system. The date of inspection was listed as 12/1/11 and the tag is
from HFP Fire Sprinkler Corporation.A call to this company revealed they have not been
there since October of 2009 and the account was on a freeze due to the lack of payment.
HFP had changed names in 2010 and no longer uses those tags.
My obvious question is who signed off on the inspection tag? I was pursuing legal advice
from the State Fire Marshals Office when you called and informed me that you had the
system inspected on 12/28/2011 and had forwarded the proper paperwork.At that time I
informed you of the discovery,and you indicated that the maintenance man might have
signed it off.
At this time I will not pursue these code violations and possible criminal act.You should
know,however,that I could have fined you $400.00 for these violations and required a
"Show Cause" Hearing on your liquor license.

Due to the history of code violations,the Yarmouth Fire Department hereby orders
Anthony's Cummaquid Inn to have all systems inspected, cleaned and ready for the annual
liquor license inspection by December lst of each year. Inspection reports from the various
vendors will be required at this inspection.
In addition,the 12/28/2011 Sprinkler Inspection Report from SimplezGrinnell indicates the
following current violations:
1. Electric Be1UWater Motor Gong not working.
2. Two water pressure gauges need replacing.
3. 5-year internal inspection needed.
4. Replacement of 28 sprinlcler heads required.
5. Coverage needed to basement stairway and an engineering study needed for enclosed
patio(4000 Sq. Ft.).
Please respond to the Yarmouth Fire Department Fire Prevention Office in writing of a
Plan of Action to correct these items by 1/31/12 with a completion date of 3/15/12.

Hello Michael,
Please see attached. This is disturbing, I did not see any improvements over last years inspection and
think things are worse from fire prevention and health standards. I am therefore notifying all town entities
involved in inspecting and licensing the building.
I need the written plan of attach to accomplish those items not accomplished last year, once a plan is
provided we will discuss the timeframe for achieving the necessary corrections.
I also need a current inspection report on the Fire Alarm System.
I will ask the powers to be to not allow occupancy in the upstairs dwelling until all items are corrected.
A further written order will be forthcoming with citations and possible fines.
