I ha.ve reviewed the status of all applications from establishments that receive permits from the
Yarmouth Board of Health. According to Federal, State and Local laws, your establishment must
have valid Permit(s) in order to remain open, insurance and liability issues notwithstanding,
At this date, no application has been submitted from your estabGshment.You are currently
operating without a permit.
Therefore, please submit an application for your establishment with the accompanying pertinent
certifications
no later than Auri123,2004.
If the Yarmouth Health Department does not receive your application by this date, a Board
of Health Hearing will be scheduled for Mav 3, 2004. At tlus Hearing your Permit(s) may
be Modified, Suspended or Revoked and fines may be issued.
