Re: 2001 and 2002 Permits
Dear Ms. Johnson,
Please be advised that we have not received applications for renewal of your establislunent's lodge
permit and continental breakfast pernut for either the years 2001 or 2002. Without these permits,
your establishment, Beachberry Inn at 157 Berry Avenue, West Yarmouth, is operating in violation
of the provisions of Massachusetts General Laws.
Within five (5) days receipt of this notice, you must come into the Health Office to complete the
application for the above licenses. The cost is$50.00 for a lodge pernut and$35.00 for a continental
breakfast permit. (Total cost of pernutting for 2001 and 2002 equals $170.00.) Please note that
Worker's Compensation information is alsc�required at time of application. You must also state on
the application who is the onsite manager.
You must call to schedule a full Team Inspection as well.
Failure to comply with the above orders will result in further Board of Health action,
