This office has reviewed the list of licenses granted to the businesses in the Town of Yarmouth, and
has noted that, as of this date,your business has not returned its application for the 2001 permits.
Enclosed please find another application and Worker's Compensation Affidavit to be filled out.
Please submit the completed documents to this office within ten(10) days of receipt of this notice.
Failure to comply may be considered a violation of State code,resulting in a possible administrative
hearing to discuss the future of your business and permits.

Friday.
