� Dear Mr. Conrad:
I This letter is in regard to the swimming pool located within the Blue Rock Heights Association property. As you
are aware, on June 21, 19y9,the Board of Healtli heardyour request fcr a:vari�nce from*..he Bc�ard's swimming pool
regulations, Safety Training Program,which required one staff inember to be available on the premises during pool
operating hours. Two staff inembers are required to have CPR,first aid and water safety training.
The Board of Health granted a variance from the pool regulation provided that the variance was requested every
year in writing and an annual report will be submitted that contains answers to the following questions:
1. Is access limited to owners and guests only?
2. Are gates locked at all times, either keyed entry or combination? Are gates self-closing and self- latching?
3. Has a "Swimmers Log" of daily bather load been maintained during the season and the results submitted
(with the next year's application)to the Health Department?
4. Has a"Safety Log" of any reported accidents(if any)been maintained during the season and the results
submitted(with the next year's application)to the Health Department?
5. Has the Certified Pool Operator(CPO)supervised chemical and physical maintenance of the pool and signed
the "Daily Pool Log" for chemical balance on a weekly basis?
6. Was the water chemistry tested for balance four(4)times per day of use?
7. Was bacteriological testing for coliform bacteria and Pseudomonas performed by an accredited lab
before the pool was opened? Were the results acceptable? If not, was there immediate remediation with a
follow up sample tested? Was testing done again in August as per State Code?
8. Was a formal inspection by the Yarmouth Health Department along with the CPO performed before the
pool was opened?
�. Is all.first aid and safety equipment as described in the State Code present on site and readily available?
10. Were all safety signs posted as reyuired by the i own of Yar�tr�ut�^
11. Were all safety signs posted as reyuired by the State of Massachusetts?
12. Was the pool in compliance with all other locate and state Swimming Pool regulations?
� � You are hereby requested to appear at the Board of Health meeting on Monday, May 6, 2013 at 5:00 PM in the ��
��� downstairs hearing room. At this time,you will need to inform the Board of your pool operation as outlined in your �

annual report. Please contact the Health Department upon receipt of this letter to confirm your attendance at the ;1
� Board's meeting and submit your annual report to the department by April 30,2013. __..%'
