Dear Mrs. Baker:
I located information, at your request, that was on file at this
department with regards to the site referenced above.
� The 1974 "as built" card on file in the Health Department indicates,
that the components consist of those subsurface septic components
required by the State of Massachusetts Minimiun Requirements for the
subsurface Disposal of Sanitary Sewage, as outlined in Title V sec.
310 CN� 15.00.
This department has no record of any septage pumping maintenance at
the site referenced above. This department reco�nends septic systems
have a maintenance pumping every 4 years.
The information supplied by this office does not guarantee that the
system will function satisfactorily.
If you should have any questions or coitments relative to this matter,
