Dear Mr. Hynes,
During a routine food establishment inspection, performed on Thursday, June 4, 1998, it came to my
attention that the Cape Cod Irish Village is selling cigarettes. In order to sell any tobacco products
in the Town of Yarmouth, a tobacco permit must be obtained from the Yarmouth Health Department.
The fee for the pennit is$20.00 and must be submitted to the Health Department within five (5) days
of receipt of this letter.
Once your fee has been obtained, I will perform a routine tobacco inspection to make sure your
restaurant is in compliance with Yarmouth's Tobacco Control Regulations. I have enclosed a copy
of the regulations for you to review. I have also enclosed a11 the necessary forms and signs you need
to keep at the restaurant.
You will need to obtain a Tobacco License from the Department of Revenue as well. Contact the
Department of Revenue in Hyannis to obtain an application.
ffyou should have any questions or comments relative to this matter, please contact me at the Health
Office. I can be reached by calling(508)398-2231,e. 240, Monday through Friday, from 8:30 a.m.
to 4:30 p.m.
