The submitted plan proposes ta relocate two(2)leaching pits outside of an e�usting Zone II � � of a public water supply we1L The Department hereby approves the modification to the wastewater treatment facility as shown on the above mentioned engineering report and plans subject to the following: � i 1. The Wastewater Water Treatment Facility as well as all pumping,ejector or lift stations ` shall adhere to all terms and conditions of Groundwater Discharge Permit 344-4 and any i other prior approvals not superseded by this modification. ' 2. Construction shall be in strict conformance with the submitted engineering report, r engineering plans cited above and provisions of this approval.No changes shall be made � without the prior written approval of the Department. � 3. Subsurface components of the system shall not be backfilled or otherwise concealed from 3 view until the Department has conducted an inspection and permission has been granted by the Department to backfill the system. 4. Operation and maintenance of the facility must be in accordance with 314 CMR 12.00: "Operation and Maintenance and Pretreatment Standards for Wastewater Treatment Works and Indirect Discharges" and 257 CMR 2.00: "Rules and Regulations for Certification of Operators of Wastewater Treatment Facilities". ', 5. The referenced facility has been rated by this office(in accordance with 257 CNIR 2.00)and � it has been tentatively detemiined that the proposed wastewater treatrnent plant is a Grade 4 � facility. For official approval we will forward this grading to the Board of Certification of � Wastewater Trea.tment Plant Operators.Therefore,please be advised that as a Grade 4 � facility,it is required that the person in responsible charge(Chief Opera.tor)possess a valid Grade 4 Massachusetts Wastewater Treatment Operator's License. Also,a backup operator must be provided who sha11 possess at least a valid Grade 3 license. 6. The operation of the referenced facility must comply with all the requirements listed in the ' Groundwater Discharge Permit 344-4. 7. This approval pertains to the discharge of sanitary waste only.Non-sanitary waste shall not , be dischazged into the proposed wastewater treatment facility. 8. The owner/operator of the system shall properly operate and maintain the system at a11 times in accordance with this approval,submitted design report and soil absorption system plans cited above. 9. The owner/operator shall fiunish the Department,within a reasonable time,any information, f which the Department may request to determine whether cause e�sts for modifying, 3 revoking,reissuing or ternunating this approval or to determine whether the owner/operator ! is complying with the terms and conditions of this approval. ' 10. The facility served by the system and the system itself shall be open to inspection by the Department at all reasonable times. . 11. As built plans of this modification to the wastewater treatment facility sha11 be submitted to ' this office of the Department within ninety(90)days of the Department's authorization to '� operate the facility as modified. I 12. The Department must approve in writing any expansion of the facility resulting in additional sewage flows to the wastewater treatment facility.
