� clearance criteria is 1500 spores per cubic meter of air in non-living spaces (basements, crawl
; spaces, etc.) and 1000 spores per cubic meter of air for living spaces. This is for spore types of

concern and is the level above the ambient outdoor air. After the re-clean of the crawl spaces and
the upstairs hallway all samples have now passed our clearance criteria. The final report

� DESCRIPTION
' At your request a representative of Target Inspections certified in Microbial Investigations and
�
Assessments visited 85 MA Route 28 in Yarmouth, MA on August 23�d and 24t", 2016 and
September 2"d, 2016 to perform a Post Remediation Verification (PRV) microbial clearance
� inspection. The scope of the PRV inspection included a visual inspection and air sample
i collection in the following areas of the apartment complex:the basement,dirt crawlspace,
� concrete crawlspace,the first floor of the affected area, and the second floor of the affected
area.The aforementioned areas were affected by water damage and microbial contamination
due to a burst pipe in the ceiling cavity of the 2"d floor. An external control sample was
collected to compare outside versus indoor spore concentrations for each sampling episode. AII
items were utilized, as well as laboratory results,to determine if microbial remediation /clean-
� up performed was successful.
A visual inspection of the areas was performed as well as moisture content, humidity, and
temperature readings. In addition to the visual inspection, air samples were collected in the
affected areas of the complex for review by approved laboratory technicians.

� One (1) Deluxe Air Sampling Kit with a MegaLite Pump was utilized in the collection of air
samples (Serial Number 14250). Air flow was measured with a rotameter calibrated in February
of 2016 with a Dri-Ca)flow calibrator. Industry standard Allergenco-D air cassettes (Expiration
� dates of luly of 2017)were utilized to collect airborne samples from inside and outside of the
home.These samples were hand delivered to H20 EnviroComp in West Dennis, MA for analysis
where analysts certified by the McCrone Research Institute in Chicago, IL analyzed the
aforementioned samples.A summary of the results of these analyses have been included in this
report below including the Chain of Custody for the samples.
Additional equipment used for analysis include—
Extech—Moisture& Humidity Meter—s/n 10016037(Passed Salt Calibration 8/1/2016)
Tramex MRH—non-invasive moisture meter—Serial Number: MRH 11091306 (Passed

collected as determined by a complete visual assessment of the "client defined" area
before mold can be identified in designated area or areas.
• The results of limited mold sampling are not a guarantee that mold does or does not
exist in the other areas of the subject property. The results are indicative only of the
presence or absence of mold in the selected areas sampled at the time the limited mold
; sampling was performed.

2nd Floor Room 14=Air sample collected approximately in the center Apartment 14 on the 2nd
floor just off of the source (air sample).
Control = External ambient control samples collected at the side of the building(air sample).
AIR SAMPLE RESULTS (Please see laboratory results included below in the report):
The laboratory findings indicated the folfowing-
- LOW/ACCEPTABLE levels of airborne Aspergillus/Penicillium-Like,Ascospores,
Cladosporium, and Basidiospore, Chaetomium, Ganoderma, Curvularia,Alternaria,
Pithomyces, and Smut spores within all indoor samples collected compared to the
confirol sample. These levels of contamination are below recommended action levels
and/or within parameters of the externa/ambient control sample. No further action is
required.
Additional remediafiion is only recommended when indoor air quality samples have spore
concentrations both above external ambient levels and above the recommended action level of
1000 spores per cubic meter for indicator spores in living areas and 1500 spores per cubic
meter of air in non-living areas (such as crawlspaces)or if visible mold/debris remain in the
remediated area(s). If these criteria have not been met then No Additional Remediation is
required. Remediation may be recommended if spore counts in conjunction to Wisual inspection
indicate the potential for a future problem or other considerations that may need to be
addressed. The only exception to this guideline is Stachybotrys,there are no acceptable levels
of this mold spore within a home or building. ,
NOTE-there is always a potential health risk to children, elderly, people with suppressed
immune systems or people with allergies to specific contaminants even with microbial spore
counts below the recommended action levels.
SUMMARY&CONCLUSIONS
Additional remediation actions were performed as a result of the August 23�d and 24th, 2016
inspections and sample collection/analysis. Visual inspection of the remediated areas indicated
no areas of visual microbial contamination and acceptable levels of airborne microbial
concentrations in all of the inspected areas as of the September 2, 2016 inspection. Sample '
collection and laboratory analyses confirms that the indoor air microbial spore concentrations
are below recommended action levels and/or below external ambient levels as described
above for a successful response action. Humidity within the inspected areas were within the
EPA recommended levels of 30%-60%, including the basement level. Microbial remediation was
performed by the Mold Doctors (572 MA-28, West Yarmouth, MA)and facility staff performed ,
demolition of various water damaged building components. Demolition was performed in, but ;

may not have been limited to,the hallway stair area on the first floor,the common kitchen area
on the first floor,the apartment adjacent to the kitchen (#2),the second floor hallway (and
apartments off of this hallway), and the basement. No other conditions were observed that
wouid lead to future mold growth, as of the time of testing.

Standard of Care
Mold Doctors will provide all materials, equipment, and labor for mold a remediation. Mold
Doctors will create full isolation of the work area during the remediation process. Mold Doctors
will create negative air pressure to contain mold levels within the work area. Mold doctors will
remediate the mold back to normal fungal ecology. Mold Doctors will not be responsible for any
mold re-growth if the original reason for the mold has not been address: moisture,water issue,
leaking pipes, no dehumidifier to control humidity, inadequate ventilation, improper venting,
roofs leaking etc.
� Mold Doctors guarantees all work after the remediation is complete. iVlold Doctors
guarantees normal fungal ecology on all mold work. If normal fungal ecology is not present after
the remediation the Mold Doctors will be responsible to achieve normal fungal ecology at no
cost to the customer
