GENERAL NOTES:
1. THE PURPOSE OF THIS PROJECT IS TO RECONSTRUCT AN EXISTING FAILED TIMBER BULKHEAD
2. THE EXISTING TIMBER BULKHEAD IS PERMITTED UNDER LICENSE PLAN NO. 1298 & CHAPTER 91 LICENSE (11298) FROM THE DEPARTMENT OF
ENVIRONMENTAL QUALITY ENGINEERING. THE BULKHEAD WILL BE RECONSTRUCTED IN THE EXISTING FOOTPRINT.
3. SUPPLY MATERIAL., EQUIPMENT AND LABOR TO INSTALL THE PROPOSED WORK ALONG THE SHOREFRONT AS DESCRIBED AND SHOWN ON THE PLAN
AND DETAILS.
4. PERFORMANCE OF THE WORK SHALL BE IN COMPLIANCE WITH THE PLANS, SPECIFICATIONS, AND ORDER OF CONDDIONS ISSUED BY THE
YARMOUTH CONSERVATION COMMISSION FOR THE REFERENCED PROJECT AND AS DESCRIBED BELOW.
5. ACCESS TO BE BOTH FROM THE UPLAND OF THE APPLICANT'S PROPERTY, AND FROM A WORKING BARGE WORKING BARGE NOT TO BE LOCATED
OVER MARSH AT ANY TIME STAGING AREA FOR MATERIALS TO BE IN DRIVEWAY OR ON BARGE
6. METAL TO BE HOT DIPPED GALVANIZED AFTER FABRICATION.
7. SHEETING MATERIAL SHALL BE SHOREGUARD 750. MINIMUM SHEETING LENGTH SHALL BE 16'.
8. THE EXISTING ANCHOR SYSTEM WILL BE UTILIZED. THE EXISTING TIE BACKS WILL BE BROKEN AT A POINT DETERMINED N THE FIELD, AND THEN
SHALL BE WELDED TO PROPOSED CHANCE SS -5 ANCHORS WITH 1-1/2' DIAMETER TIE RODS MINIMUM. THE PROPOSED WALL REPLACEMENT IS
DESIGNED FOR 6' EXPOSURE THE ANCHOR FORCE IS 4.9 KIPS. ANCHORS SHALL BE TENSIONED TO CALCULATED ANCHOR FORCE MINIMUM THEN
BACKED OFF AS NECESSARY TO STRAIGHTEN WALL. ENGINEER SHALL BE NOTIFIED IMMEDIATELY IF MINIMUM DESIGN SPECIFICATIONS CAN NOT BE
ATTAINED. A TORQUE LOG SHALL BE KEPT BY CONTRACTOR DURING INSTALLATION OF ANCHORS AND GIVEN TO ENGINEER AT COMMENCEMENT OF
INSTALLATION.
9. CONSTRUCTION DETAILS TO BE APPROVED BY ENGINEER PRIOR TO CONSTRUCTION.
10. DISTURBED AREAS TO BE REVEGETATED M MATCH PRE—CONSTRUCTION CONDITIONS.
11. CONTRACTOR TO VERIFY ALL DIMENSIONS IN THE FIELD AND NOTIFY ENGINEER OF ANY DISCREPANCIES PRIOR TO CONSTRUCTION.
12. ANY CHANGES TO ANCHORS AND SPECIFICATIONS SHALL BE APPROVED BY THE ENGINEER IN WRITING PRIOR TO IMPLEMENTATION.
