CONSTRUCTION NOTES:
1. ELEVATIONS REFER TO NGVD. CONTACT SHOREFRONT CONSULTING FOR ADDITIONAL ELEVATION
INFORMATION AS NEEDED.
2. CONSTRUCTION ACCESS SHALL BE VIA THE ACCESS ROUTE SHOWN ON THE PLAN AND DIRECTLY
OVER THE APPLICANT'S PROPERTY.
3. THE WORK ACTIVITY SHALL OCCUPY A MINIMUM AREA TO ACCOMPLISH THE INSTALLATION OF THE
WALKWAY AND PIER. EQUIPMENT AND MATERIALS HANDLING SHALL BE IN ACCORDANCE WITH THE
WORK PROTOCOL SUBMITTED WITH THE NOTICE OF INTENT, THE RESULTING ORDER OF CONDITIONS
AND AS DIRECTED BY THE YARMOUTH CONSERVATION AUTHORITIES.
4. ALL WORK SHALL BE ACCOMPLISHED FROM A WORKING BARGE AS MUCH AS PRACTICAL. THE
REMAINDER OF WORK SHALL BE ACCOMPLISHED BY HAND. AT NO TIME SHALL THE WORKING BARGE
COME IN CONTACT WITH THE POND BOTTOM.
5. EQUIPMENT SHALL BE STORED ON THE APPLICANT'S PROPERTY WHEN NOT IN USE. WORKING BARGE
TO BE ANCHORED TO AVOID BOTTOMING OUT.
6. PILES SHALL BE DRIVEN TO THE DEPTHS SPECIFIED ON THE PLAN. IF DESIGN EMBEDMENT CAN NOT
BE REACHED, SHOREFRONT CONSULTING SHALL BE CONTACTED BEFORE PROCEEDING. PILES CAN BE
MECHANICALLY DRIVEN FROM THE WORKING BARGE WHERE POSSIBLE.
7. ALL AREAS DISTURBED DURING CONSTRUCTION SHALL BE RETURNED TO THEIR PRE -CONSTRUCTION
CONDITION. ALL WORK SHALL BE INSPECTED BY SHOREFRONT CONSULTING AND RECEIVE APPROVAL
BEFORE REMOVING EQUIPMENT FROM THE SITE.
8. THE ACCESS ROUTE AND WORK SITE SHALL BE CLEANED AND RESTORED TO RESEMBLE THE
PRE -CONSTRUCTION CONTOURS AND CONDITIONS.
9. THE CONTRACTOR SHALL OBTAIN A COPY OF THE ORDER OF CONDITIONS ISSUED FOR THE PROJECT
BY THE YARMOUTH CONSERVATION COMMISSION PRIOR TO THE START OF ANY WORK.
10. ALL MATERIALS IN CONTACT WITH THE GROUND SHALL BE PRESSURE TREATED. ALL TIMBER USED
FOR STRUCTURAL FRAMING SHALL BE PRESSURE TREATED, ACQ, OR EQUAL AS APPROVED BY OWNER
OR SHOREFRONT CONSULTING.
10. ALL HARDWARE SHALL BE GALVANIZED OR STAINLESS STEEL, UNLESS OTHERWISE APPROVED BY
OWNER OR SHOREFRONT CONSULTING.
12. THE PROPOSED PIER IS PERMANENT. THE RAMP AND FLOAT ARE SEASONAL.
13. THE FLOAT SHALL BE STORED ON THE APPLICANT'S UPLAND AREA DURING THE WINTER MONTHS IN
THE LOCATION SHOWN ON THE PLAN.
