Attached is the Town of Yarmouth license renewal application and workers compensation insurance affidavit for 2017.
Please print out the forms,complete them fully,and return them to our office with the$125.00 payment at your earliest
convenience.
Please remember to attach current copies of ServSafe,and Allergen certifications.
Please note that your current Yarmouth Food Service License expire December 315t
