Oceanside Septic, Inc. completed the final inspection for the newly installed sewage disposal
system at 6 Jacqueline Circle, Yarmouth, MA. The sewage disposal system consisted of a 1500
gallon septic tank, an H-20 Rated DB-6 Distribution Box and twenty five (25) ADS leach '�
chambers in bed configuration. �
The subsurface sewage disposal system installed at the above referenced property has been ,
constructed in substantial compliance with 310 CMR 15.000, all local requirements of the
Yarmouth Board of Health, and the approved design plans dated 10/10/14 and revised 10/26/15 ;
as filed with your office. ;
