Dear Bruce,
As a follow-up to my letter of July 31, 1998, on August 12, 1998 I made
subsequent inspection of the d-box and found the d-box had been replaced with a
n�a d-box. The new d-box passed the water test.

Dear Bruce,
On this date, I made an on-site inspection of the installation of the septic system for the above
reference property and find the system to be installed in substanrial compliance with the Mass
code Title 5 and our approved plan dated March 23, 1998 with the following notes:
The cover to the d-box appears to be smail
There was insufficient water onsite to inspect the flow levelers
