The Yarmouth Board of Health, having reviewed your application on Se�tember 12, 2016 for a
variance from the provisions of Title 5 of the State Environmental Code, 15.211 (1 , 1Z 5.211
�) 15 212 (1) 15 255 (2) and 15.255 (5) andlor from the Town of Yarmouth Amendments to
Title 5, Section 3_7, for a Sewage Disposal System and, having determined strict enforcement
of these regulations in this instance would do manifest injustice and, further, that your
requested variance does not conflict with the spirit of the State Environmental Code nor the
Town of Yarmouth Amendments, has hereby granted the variance(s). The variance(s)
granted are as follows:

Variances are shown on an engineered plan prepared by EAS Surveying, Inc. dated May 18,
2016 and revised June 28, 2016 and September 23, 2016.

2. To Allow the Leaching Area to be 10 Feet from a Crawl Space Foundation in Lieu of

3. To Allow the Septic Tank to be 6 Feet from the Cellar Wall in Lieu of 10 Feet.

Order of Conditions:
1. A 1,500 gallon septic tank will be installed to replace the existing 1,000 Septic Tank.
2. There will be no additional habitable space without Board of Health Review.
