Dear Bruce,
�Ve i�ave inspected the instaliation of tlie leacfiing system at��Breezy�oint Road in
South Yarmouth. The leaching area was installed by PKM Construction in substantial
compliance with the design plan of May 18, 2016 and revised September 23, 2016, and
approved by the Yarmouth Board of Health.
Prior to construction the wall and property iine were sta�ed out.
Inspection performed the week of February 27, 2017
Inspection of overdig&wall footing
Inspection of 1,500 gallon H-10 septic tank, 650 Pump Chamber& "D"box.
Inspection of��low profile''`Quik-4"plastic units.
Inspection ofpump floats and alarm and all S.A.S. elevations.
