2. VERTICAL DATUM IS NGVD 29. FOR BENCH MARKS
SET, SEE SITE PLAN.
3. ALL CONSTRUCTION METHODS AND MA TER I AL S AND
MAINTENANCE OF THE SEPTIC SYSTEM SHALL
CONFORM TO MASS. D.E.P. TITLE 5 AND LOCAL
BOARD OF HEALTH REGULATIONS.
4. ALL SEPTIC SYSTEM COMPONENTS LOCATED UNDER
AREAS SUBJECT TO VEHICULAR TRAFFIC OR GREATER
THAN 3' IN DEPTH SHALL BE CAPABLE OF WITH-
STANDING H-20 WHEEL LOADS.
5. ALL SEWER PIPE SHALL BE SCHEDULE 40 PVC OR
APPROVED EQUAL.
6. D -BOX SHALL BE REINFORCED PRECAST CONCRETE.
D - BOX SHALL BE WATER TESTED TO CHECK FOR
LEVEL WHEN THERE IS MORE THAN ONE OUTLET.
7. EXISTING SEPTIC TANK AND PUMP CHAMBER TO BE
INSPECTED FOR ADEQUACY PRIOR TO CONSTRUCTION.
8. BEFORE CONSTRUCTION CALL "DIG -SAFE'.
1 -888 -DIG -SAFE AND THE LOCAL WATER DEPT.
FOR LOCATION OF UNDERGROUND UTILITIES.
9. SEPTIC SYSTEM INSTALLER SHALL NOTIFY THE
DESIGN ENGINEER TWO DAYS PRIOR TO CONSTRUCTION
OF THE SYSTEM TO ALLOW FOR SCHEDUL 1 NG OF THE
CONSTRUCTION INSPECTIONS.
10. ALL UNSUITABLE MATERIAL (A c4 B HORIZONS)
ENCOUNTERED BELOW THE INVERT OF, THE LEACHING
FAC I L I TY. I NCL UD I NG THE EXISTING LEACHING SYSTEM
SHALL BE REMOVED FOR A DISTANCE OF 5' AROUND AND
REPLACED WITH SAND IN ACCORDANCE WITH TITLE 5.
11. ALL PROPOSED CONSTRUCTION ACTIVITY IS LOCATED
WITHIN THE AREA OF MINIMAL FLOOD HAZARD. ZONE X
