Under the terms of this � maintenance agreement, an Oakson, Inc. service technician will check the operation of your � Drip Dispersal Septic System at regular intervals outlined herein. The contract price will ; remain in effect for two years, as listed above. Semi-Annually Inspection: : ; � • A visual check and walk-through of the Drip Dispersal System E • Check control panel lights and switches • Check that pin lights are operating correctly • Verify output operation in automatic mode of control panel � Check ptunp chamber liquid level and float switches 2. Annual Inspection to inciude semi-annual tasks as well as all items listed below: ' • Verify proper operation of air release valves • Pressurize system and check dosing flaw rates to each zone • Check flow meter operation. The following terms apply to this agreement: _ ( • Parts and labor costs shall apply for the repair or replacement of any equipment ! which has failed due to flooding, neglect, or for exceeding design limits as set forth in the wastewater system design. Water infiltration into tanks, conduit, or items � installed by others is not covered in this agreement. Infiltration can include - but is not limited to - groundwater, rain, melting snow, swales, running toilets/faucets, or water treatment systems • Parts and labor costs shall apply to all equipment which has not been properly maintained or equipment which has been disassembled, repaired by unauthorized persons,subjected to external damage, or otherwise mistreated � • Parts and labor after the one-year American Manufacturing Company Inc. warranty has expired will be billed to the owner for any repairs required • Parts and labor directly related to the lack of proper Drip Dispersal System � maintenance shall be billed to the owner ' i ; • The owner shall be billed for the cost of pumping and hauling of waste. The Pero-Rite Drip Dispersal System was installed at 94 Iroquois Boulevard by Bryan Barrows. All components including the hydraulic unit,pump, floats,control panel, supply and retuxn lines,and the drip tubing itself were installed to the manufacturer's specifications. System start-up commenced on October 22,2008 and was functioning appropriately at , that time. Viewing installation of a Perc-Rite Drip Dispersal is usually the best way to get a hands-on feel for how easy it is to build and bene�cial it can be to use for a property. There has been an interest from a number of folks on Cape Cod to see a drip dispersal system in the ground. There is currently a system being installed in the town of West Yarmouth, Massachusetts which we are able to make open for viewing to you. We have this opportunity on Wednesday,October 22nd, 2008 from 1-2 at 94 Iroquois Boulevard,West Yarmouth. Iroquois Boulevard is located off South Sea Avenue which is off Route 28. We hope you can visit the site and see the installation and operatian of a Perc Rite Drip Dispersal System. If you do visit please stay within the window of time announced. We apologize for the short notice but we hope you will able to fit this into your schedule. Perc-Rite Drip Dispersal was selected at this site in order to reduce the�nished height. There is going to be a small rise above the previous ground surface due to the high water table, but this has been rninimized due to the use of the Perc-Rite system. Additionally, the lack of the required 5'overdig and the placement of the tubing at 1'on-center has greatly reduced the area of disturbance on the parcel. The project is an upgrade at a single-family home using septic tank effluent going out to the drip dispersal field.
