Status on revisions and completed Deed Restriction application? I did receive a floor plan from the realtor
verifying the existing 2 bedrooms. I wasn't sure if 1 missed something in my office.

Hi Jason-
1 have reviewed the plan and have the following comments to address prior to approval:
1. Provided inspection port in leach field.
2. Add/show check valve in force main in pump chamber.
' 3. Specify pipe sizes being proposed.
4. Cap or connect ends of leach field laterals.
5. Specify size of weep hole in pump chamber.
6. Provide notes: alarm on separate circuit from pump, electrical permit required.
7. On leach field detail, show liner around entire perimeter of leach field.
8. Revise the Variance request#3. My interpretation is the separation distance only needs to be 25'to
basin so variance is only 2'. (Our local regulation requires 100' separation if the drainage system
empties into a wetland and this is a closed system that does not drain beyond the leach portion of it.)
9. Provide wall specs.
10. Need a floor plan.
11. Need a completed Title 5 Deed Restriction Application.
Please call or email with any questions or comments. I will need 3 revised copies and the deed restriction
application to process the installation permit.

I Hi Jason-
Skip Phelps has spoken with me on behalf of the owner regarding making some changes to the design. The
owner would like to reduce the height of the mound by requesting a 1' variance to groundwater and to change
the retaining wall to a block wall on the northerly and westerly side of the leach field only. The remaining two
sides of the field can be graded back towards the house and the driveway beyond the 40 ml liner used for
breakout.
We will grant the variance to groundwater in-house. The property will need a two bedroom deed restriction per
Title 5 because of the 2 bedroom size leach facility. The design engineers usually complete the application for
the owner and return it to our office for processing. I have attached a blank form for your use or to forward to
the owner.
