As per your request and in accordance with 310 CMR 15.021 (3)of the D.E.P. State Environmental Code,
Title 5,I am addressing this correspondence to you directly regarding the above captioned project. I hereby
certify that the Subsurface Sewage Disposal System has been installed in substantial Compliance with the
Design Plan(16-001)by Moran Engineering Associates,LLC.
NOTE:backfilUgrading over the septic system tank was not completed. The(modular)dwelling was being
placed on the foundation and the tank required being exposed to protect it from being subject to heavy
equipment.
