Massachusetts Youth Soccer Camps have been
:. � -� . designed to follow US Soccer best practices for
" player development. The camp curriculum will follow

each player's performance throughout the duration

- - All of our camp staff hold US Soccer National Licenses
- and have undergone extensive training on how to
improve youth soccer players using empowerment
and inclusive training methodology.
i�1assachusetts Youth Soccer wiil partner with
Challenger Sports to offer these programs to players
in the Massachusetts soccer community. All camp
- - participants will be provided with a uniyue environment
��- , ::: � that will develop their technical and tactical soccer skills

- mastery, small and large sided games.

Attach the names, ages, applicable current certifications (if any), such as First Aid, and
the anticipated role at the camp of all supervisory staff(see below). Use as many pages
as necessary to complete this.
Please: If you are applying for an original camp license for a camp based in Yarmouth,
you must file a plan showing the following with the board of health at least 90 days
before your desired opening date (See MGL Ch. 140 § 32A):
➢ Buildings, structures, facilities and fixtures
➢ Proposed source of water supply
➢ Works for disposal or sewage and waste water
Superviso staff ineans those persons with the responsibility, authority and training to
provide direct supervision to camper groups. This may include counselors, junior
counselors, general activity leaders or other staff who provide supervision to campers �
without assistance.

Please fmd enclosed the Challenger Sports 2017 Board of Health Packet. Within this
packet you will fmd our Policies and Procedures, copy of our 2017 Insurance Certificate,
Health Care Consultant Agreement, Written Orders from our Health Care Consultant and
our application for the camp license itself, including the application fee.
Please note,this packet does not contain the following documents/information:

• Staff's physical and immunization records
• Camper records(physicals and immunizations)
• Camp Director/Camp Health Supervisor(Director/Health Supervisor is allocated
a week prior to camp)
All information regarding our Challenger Sport Coaching staff (CORI, SORI, and
physical and immunization records) can be presented prior to the week of camp, or
during the week of camp respectively. The collection of all camper information(physical
and immunization record)is an ongoing process.
Should you needlwish to review any and all information,please let me know a date when
that could be arranged?
