Good morning.
We have received a 2018 renewal application for Sea Dog Brew Pub, however,the payment fee submitted for licenses
was not correct.The application which was completed and submitted to our office only had the Food Service fee
checked off and paid for. (It was not the application which was attached in the original renewal request email.)
Please note that the Common Victualler license fee of$60.00 is still owed.
Would you please submit the$60.00 fee at your earliest convenience?

To Food Service Establishments(greater than 100 seats):
Attached is the Town of Yarmouth license renewal application and workers compensation insurance affidavit for 2018.
Please print out the forms,complete them fully, and return them with any required certifications to our office with the
$260.00 payment by December 15, 2017.
Please note that your current Yarmouth Retail Food Service License expires December 315t
