GENERAL NOTES;
1. ALL CHANGES TO THIS PLAN MUST BE APPROVED BY THE LOCAL
BOARD OF HEALTH AND THE DESIGN ENGINEER.
2. ALL WORK AND MATERIALS SHALL CONFORM TO THE REQUIREMENTS
OF THE STATE ENVIRONMENTAL CODE, TITLE V, AND ANY APPLICABLE
LOCAL RULES AND REGULATIONS.
3. THE SEWAGE DISPOSAL SYSTEM SHALL NOT BE BACKFILLED PRIOR
TO INSPECTION AND APPROVAL BY THE BOARD OF HEALTH AND THE
DESIGN ENGINEER.
4. ANY CONDITIONS ENCOUNTERED DURING CONSTRUCTION DIFFERING
FROM THOSE SHOWN HEREON SHALL BE REPORTED TO THE DESIGN
ENGINEER BEFORE CONSTRUCTION CONTINUES.

IN THE AREA BENEATH AND FOR 5 FT. ON ALL SIDES OF THE S.A.S.
AND REPLACE WITH CLEAN FILL AS SPECIFIED IN 310 CMR 255(3).
12. CONTRACTOR SHALL EVALUATE STUCTURAL INTEGRITY OF EXISTING
SEPTIC TANK PRIOR TO CONSTRUCTION.
