The current exemption for `Homeowner' was extended to include owner—occupied dwellings of one or two units
and to allow such homeowners to engage an individual for hire who does not possess a license,provided that such
homeowner shall act as supervisor. (State Building Code Section 110 R5.1.3.1)
Definition of Homeowner.
Person(s)who owns a parcel of land on which he/she resides or intends to reside,on which there is or is intended to
be, a one or two family attached or detached structure assessory to such use and/or farm structures. A person who
constructs more than one home in a two-year period shall not be considered a homeowner,such"homeowner"shall
submit to the building official,on a form acceptable to the building official,that he/she shall be responsible for all
such work performed under the buildine permit. (Section 110 R5.1.3.1)

• ' Mas,sachusetts General Laws chapter 152 requires all employers to provide workers' compensation for their employees.
Pursuant to this statute,an employee is defined as"...every person in the service of another under any contract of hire,
express or implied,oral or written."
An employer is defined as"an individual,partnership, association,corporation or other legal entity,or any two or more
of the foregoing engaged in ajoint enterprise,and including the legal representatives of a deceased employer,or the
receiver or trustee of an individual,partnership,association or other legal entity,employing employees. However the
owner of a dwelling house having not more than three apartments and who resides therein,or the occupant of the
dwelling house of another who employs persons to do maintenance,construction or repair work on such dwelling house
or on the grounds or building appurtenant thereto shall not because of such employment be deemed to be an employer."
MGL chapter 152, §25C(6)also states that"every state or local licensing agency shall withhold the issuance or
renewal of a license or permit to operate a business or to construct buildings in the commonwealth for any
applicant who has not produced acceptable evidence of compliance with the insurance coverage required."
Additionally,MGL chapter 152, §25C(7)states"Neither the commonwealth nor any of its political subdivisions shall
enter into any contract for the performance of public work until acceptable evidence of compliance with the insurance
requirements of this chapter have been presented to the contracting authority."
Applicants
Please fill out the workers' compensation affidavit completely, by checking the boxes that apply to your situation and,if
necessary, supply sub-contractor(s)name(s), address(es) and phone number(s)along with their certificate(s)of
insurance. Limited Liability Companies (LLC)or Limited Liability Partnerships(LLP)with no employees other than the
members or partners, are not required to carry workers' compensation insurance. If an LLC or LLP does have
employees, a policy is required. Be advised that this affidavit may be submitted to the Department of Industrial
• Accidents for confirmation of insurance coverage. Also be sure to sign and date the affidavit. The affidavit should
be retuned to the city or town that the application for the permit or license is being requested,not the Department of
Industrial Accidents. Should you have any questions regarding the law or if you are required to obtain a workers'

Dear Frank,
The renovations for this house have been evaluated according to your drawings and the requirements
of the 9th edition of the building code for wind exposure B, and the WFCM guide(wood framing
construction manual).
General
The cathedral ceiling for this 24 ft. span requires a 5 '/x 18 Microllam LVL structural ridge beam
as shown in sheet 13. The support structure for this beam includes beaming across openings at
each end with 5 '/x 11 7/8 Microllam LVLs,as shown in sheet 13. In each case,these beams
may be made up of three members, each 1 ' in. wide as shown in sheet 32,Assembly B. The
members are to be connected with two rows of in.dia. A307 through bolts at 16 in. o.c.
Hardware is to be dry or zinc coated but not waxed and with washers on each side. Each bolt is to
be torqued to 24 ft. lbs.,then each bolt re-torqued.
All other members and connectors supporting the ridge beam are also to be as shown in sheet 13.
The main beam girt is to be a triple 2x8 with support columns located 6 ft. apart as shown on the
drawings. The three columns supporting these beams are to be 3 '/in.dia. standard lally columns,
concrete filled,and are to be connected to the main beam girt with Simpson LCC4.5-3.5
Connectors. Footings for the columns are to be 20 in. min,x 20 in. min. x 10 in.min. deep with
no rebar. The columns are to be tight fit on steel base plates 8 in. x 8 in.x %in.thick on the
footings and each secured with two 7 inch long Hilti bolts into the footings and centered with one
at each side of the columns in the direction of the beam above. The columns are to be welded to
their base plates with 1/16 in. fillet welds,all around.The base plates may be embedded into the
concrete for flush surfaces if desired.
Deck joists are to be 2x8s at 16 in. o.c. The deck outer band supported with three equally spaced
piers is to be a double 2x10. The middle pier is to be a"Big Foot"BF 24 with a 10 in. dia.tube.
The corner piers are to be 14 in. dia. sonos.
Anchor bolts are to be 5/8 in. dia. with 3 in. x 3 in. x '''A in.thick square washers, spaced 72 in.
max. and 6 to 12 in. from ends of sill plates.
For sheathing,the structural paneling is to be %2 cdx plywood. The minimum length of full height
sheathing for the external wall width is to be 55%and for the external wall length is to be 55 %.
Nailing of the sheathing is to be with 8d common nails, 3 in. edge and 6 in. field.Nailing is to be
according to the schedule shown in Table 2 of the WFCM guide.
Simpson H2.5A connectors are to be provided at all rafter and upper plate intersections.
Analysis
Wind load selection is based on based on roof pitch,wall and roof surface area, and area section
location. The roof angle for the renovated roof is 24.94 degrees. Maximum horizontal wind load for
this angle is 29.1 psf. This resolves to a vertical wind loading of 11.13 psf. Horizontal wind load for
external walls is 22.6 psf. Snow load is 25 psf. Total vertical loading on the roof consists of snow
plus %2 vertical wind and material weight. Internal floor live loading is 40 psf.Deck live load is 60
psf. All material weight is evaluated and combined in by the computer.
