COMMONWEALTH, EVEN THOUGH HE, THE LICENSE HOLDER, IS NOT THE PERMIT HOLDER BUT ONLY A SUB-
CONTRACTOR OR CONTRACTOR TO THE PERMIT HOLDER. • .
2.15.3 THE LICENSE HOLDER SHALL IMMEDIATELY NOTIFY THE BUILDING OFFICIAL IN WRITING OF THE
DISCOVERY OF ANY VIOLATIONS WHICH ARE COVERED BY THE BUILDING PERMIT.
2.15.4 ANY LICENSEE WHO SHALL WILLFULLY VIOLATE SUBSECTIONS 2.15.1, 2.15.2 OR 2.15.3 OR ANY
OTHER SECTION OF THESE RULES AND REGULATIONS AND ANY PROCEDURES, AS AMENDED, SHALL BE SUBJECT
TO REVOCATION OR SUSPENSION OF LICENSE BY THE BOARD.
2.16. ALL BUILDING PERMIT APPLICATIONS SHALL CONTAIN THE NAME, SIGNATURE AND LICENSE NUMBER OF
THE CONSTRUCTION SUPERVISOR WHO IS TO SUPERVISE THOSE PERSONS ENGAGED IN CONSTRUCTION, RECON-
STRUCTION, ALTERATION, REPAIR, REMOVAL OF DEMOLITION AS REGULATED BY SECTION 109.1.1 OF THE
CODE AND THESE RULES AND REGULATIONS. IN THE EVENT THAT SUCH LICENSEE IS NO LONGER SUPERVISING
SAID PERSONS, THE WORK SHALL IMMEDIATELY CEASE UNTIL A SUCCESSOR LICENSE HOLDER IS SUBSTITUTED
ON THE RECORDS OF THE BUILDING DEPARTMENT.

ATTENTION: Mr. Forest White
SUBJECT: New England Telephone Canopy
Old Townhouse Road, So. Yarmouth, MA
Dear Mr. White:
Confirming our telephone conversation of Friday, 2 February 1990,
and confirming visits to your office by our Field Supervisor,
Mr. Stephen Wernig and, our Sub-Contractor, Mr. Al Juriewicz, we
outline as follows:
1. Petroleum & Chemical Equipment Services was awarded a contract by
New England Telephone Company to furnish and install a 24' Wide X
24' Long Single Column Canopy at owners facility on Old Townhouse
Road.
2. As I stated to you on 2 February, 1990, we were awaiting "Stamped
Engineered Drawings" from the canopy manufacturer, J & M Industries
Inc.
3. Mr. Wernig attempted to apply for the appropriate permit (s) to
install this canopy at your office on Tuesday, 6 February 1990.
After briefly reviewing the engineering drawings, you apparently
returned the drawings to Mr. Wernig along with the following forms
which were to be completed prior to processing of the installation
permit.
3. 1 Town of Yarmouth Building Department Construction
Supervisor Form.
3. 2 Town of Yarmouth Application for a Permit to Build.
3. 3 Building permit application sign off.
3. 4 Plot Plan for Lot IIB-2, Map 077.
4. We are returning the above forms to you for your review and
approval as well as the engineered drawings from J & M Industries,
Inc. We invite your attention to the "NCEE 113521" above the
Alabama State Seal. The Manufacturer has advised us that this is a
nationally recognized association which is acceptable in all
States.

5. The Sub-Contractor who we hired to install the-canopy is a
manufacturer authorized and trained installer of canopies.
Furthermore, he has installed in excess of 150 canopies in
Massachusetts, including the City of Boston, and previously has
never been asked for a "Construction Supervisor" license number.
He does not have the license arid, at this point, we are not sure
how to satisfy this requirement.
6. Please advise us as to the amount we owe for this permit.
Mr. White, should you have any questions, please do not hesitate to
