2.15.3 THE LICENSE HOLDER SHALL IMMEDIATELY NOTIFY THE BUILDING OFFICIAL IN WRITING OF THE
DISCOVERY OF ANY VIOLATIONS WHICH ARE COVERED BY THE BUILDING PERMIT.
2.15.4 ANY LICENSEE WHO SHALL WILLFULLY VIOLATE SUBSECTIONS 2.15.1, 2.15.2 OR 2.15.3 OR ANY
OTHER SECTION OF THESE RULES AND REGULATIONS AND ANY PROCEDURES, AS AMENDED, SHALL BE SUBJECT
TO REVOCATION OR SUSPENSION OF LICENSE BY THE BOARD.
2.16. ALL BUILDING PERMIT APPLICATIONS SHALL CONTAIN THE NAME, SIGNATURE AND LICENSE NUMBER OF
THE CONSTRUCTION SUPERVISOR WHO IS TO SUPERVISE THOSE PERSONS ENGAGED IN CONSTRUCTION, RECON-
STRUCTION, ALTERATION, REPAIR, REMOVAL OF DEMOLITION AS. REGULATED BY SECTION 109.1.1 OF THE
CODE AND THESE RULES AND REGULATIONS. IN THE EVENT THAT SUCH LICENSEE IS NO LONGER SUPERVISING

Enclosed there are two views of the property at 111 Main St. ,
Yarmouthport Ma. The request for 4 views is not applicable in that those
views do not change or are not impacted in any way, by the roof repair
in question. The deleted views are the front and the west side.
Also find a sample of shingles that will be BIRD SAIL 80 WOOD tone. eR�
These will cover the upper roof and are a continuation of what already s so \
exists on that roof plane. All other roof areas will be wood (red cedar)-s°
shingles. The side walls will have wood (white cedar) shingles.
This is a re-roof of approximately 11 ft. east to west to eliminate
a ten year roof leak having exhausted all other attempts to correct the
problem both by myself and by those before me. (There were no less than
six layers of flashing/tar that predated any of my attempts. ) The
problem originated from too many roof lines joining together in too
small a space. What I have done is to continue the roof line eastward to
eliminate the problem and clean up the unsightly compound angles.
All existing roofing was left in place and therefore the
feasibility of using this space for any interior expansion, (as rumor
would have it), would require an inordinate amount of demolition inside
the house to gain a llft.X 5ft. space. All electrical wiring and heating
ducts pass through this space as well and again the work required to
move these would not justify the gain.
