We have inspected the septic system installed at 6 New Hampshire Ave.,
West Yarmouth, by Robert Our Co. and found it to be in substantial
compliance with Title 5.
Pump chamber floats were adjusted to: 29" 2Md pump & alarm
25" 1st pip
11" off
These settings differ from plan due to use of a Shorey 2500 H-20 Tank..

To whom it may concern:

HEALTH DEPT.
Please be advised that pursuant to 3 1 0CMR1 5.255(d) I inspected the construction of the
retaining wall during the placement of reinforcement for the footing and wall and at the
completion of construction.
As a result of these inspections I believe the installation meets the requirements of
310CMR15.255 and the approved plan.
