On June 14, 2001 tests were conducted on the captioned lot to determine suitability for a Title V subsurface sewage
disposal system The results of the tests and soil data were as follows:

Title V of the State Environmental Code requires the following dosage rates for percolation rate less than 5.0
munch:
Side wall area 0.74 gals/sq.ft.
Bottom area 0.74 gals/sq.ft.
For a three (3) bedroom house having plumbing fixtures and appliances in common usage, including an automatic
sequence laundry washer and a dishwasher, but not a mechanical garbage grinder, a daily sewage flow of 330
gallons is anticipated. A 343.3 G.P.D. has been provided.
It is recommended that a precast concrete septic tank having a required capacity of 1500 gallons be installed on this
lot in series with a precast concrete distribution box (3 outlets) and leaching chambers (Ref. 15.253).
Construct one (1) leaching chamber 28' x i l' by placing a row of two (2) leaching dry well units 500 gallon and
four (4) feet of stone on all sides.
This system provides for 464 sq. ft. of leaching area and a capacity of 343.3 G.P.D. using two 500 gallon H-10
leaching drywells. The enclosed plot plan indicates the location and grades of the system. The sections show the
type of construction and materials that are recommended. All piping in the system shall be 4" p.v.c., Schedule 40,
including replacement to the foundation.
There are no known or observed wells or wetlands within 100' Title V requirements (15.211) of the proposed septic
system

The finish ground grading shall conform to Title V section 15.221 (7) in such a manner that surface water runoff
shall be diverted from the house and the leaching system. The existing cesspool is to be pumped and filled with
excavated clean medium to fine sand.
All trees, top and subsoil within ten (10) feet of the leaching area shall be removed for a depth of 18".
Construction shall conform to all Title V requirements.
Contractor to notify Engineer at the time of installation to verify soil conditions.
