Remarks: Enclosed is the recent monthly reporting forms for the system at the above referenced location. The
0&M inspection form shows no equipment was replaced and the system is operating properly. The
grease trap was pumped on schedule. Laboratory test results show good treatment of the system. The
average daily flow during this reporting period was 2,733 gallons per day.
