Site is within area of seasonal and year'round homes. It lies within the 100 year flood elevation (AE
el. 11). No other resource areas or their buffer zones affect the site.

2. a. Work Description (use additional paper and/or provide plan(s) of work, if necessary):
Proposed addition within Land Subject to Coastal Storm Flowage (LSCSF). Floodzone foundation
design (by others). Proposed fill, approximately 50 c.y., desired to bring grade up in the rear yard for
aesthetic reasons and for easier access to rear yard from the back of the dwelling. Title 5 septic system
installed 2010, no changes proposed as there is no net increase in the number of bedrooms.

and accompanying plans, documents, and supporting data are true and complete to the best of my
knowledge.
I further certify that the property owner, if different from the applicant, and the appropriate DEP Regional
Office were sent a complete copy of this Request (including all appropriate documentation)
simultaneously with the submittal of this Request to the Conservation Commission.
Failure by the applicant to send copies in a timely manner may result in dismissal of the Request for
Determination of Applicability.

PROJECT NARRATIVE FOR #166 SILVERLEAF LANE, WEST YARMOUTH
The site consists of a developed 24,393+/- s.f. lot improved with a dwelling, separate garage
and dirt driveway. The site is within an area of year `round and seasonal homes.
The site is within Floodzone AE Elev. 11. The addition foundation will be about 1.3' higher
than the dwelling; there will be no basement in accordance with the floodzone regulations. It
will be on a floodzone designed foundation (designed by others).
No other Resource Areas or their buffer zones affect the site.
About 50 c.y. of fill is proposed to be placed in the side and rear yard for aesthetic reasons as
well as for easier access to the rear of the yard from the back of the house.
The construction sequence is as follows:
• Excavation work to install 4' frost wall Any suitable material excavated will be used in
the rear yard.
• Construction lumber will be stored on the site and/or delivered as needed. The
current fencing will be modified for access and remain as a work limit line, containing
construction activities. All run-off shall be contained on-site.
• Fill will be brought on site as needed to the rear
• Disturbed areas to be loamed and seeded to match existing.
The site is not located within Priority or Estimated Habitat for Endangered Species.
