This septic upgrade will require a 2 bedroom deed restriction because of the septic size. I have approved the plan and
permit but still need a completed deed restriction application (attached) returned to me for processing. As soon as I
have the application, I can release the permit. Our office will need a copy of the recorded deed restriction prior to
issuing the COC once the system is installed. I usually mail the deed restriction directly to the owner for processing
unless I hear differently.

Hi Dan-
I have also reviewed the plan and have a couple of comments:
Li'l. Need an effluent filter in the tank outlet tee because of the pump
"/ 2. Because of the depth of the pump chamber below grade and to provide easier access for removal of the pump if
needed, either have the delivery pipe exit the riser or have the delivery line loop up into the riser and then loop
back down. Let me know if you need a template.
'e'3. Add a ball valve or a gate valve to the delivery line inside the chamber to dial down the volume of water as
needed to the dbox
C4-4. Need wall specs. If this is a poured concrete wall, it will require engineered cert.with inspections.
5. Add a note: "no abutting foundations within 20'of the leach facility" because of the property line variance
t re6. Should the house number be "86"?

Subject:83 Cottage Drive Septic Plan
Hi Dan
I have attached the standard septic review conditions to be included in your plan. Please show the work line
limit/erosion control line on the plan. I also noted that the shed is to be relocated to the back of the house. Please note
that the shed needs to be outside the 35 foot setback from the closest wetland resource area. As a minimum I need to
see it located no closer to the pond than the back side of the house. There should be enough space that it will not be
located over the septic tank.
