This project will need a 2 bedroom deed restriction. If you/owner can attempt the new process I have set up with the
fillable template on our website,that would be helpful to me to see how the template works. The form can be found on
our website under Health Department Downloadable Forms
(http://www.yarmouth.ma.us/DocumentCenter/View/10240/Deed-Restriction-Final-Yarmouth-Template-2019). There
is also a blank pdf template if the fillable form does not work for you. Once you have completed the form, Bruce will
need to sign it prior to the owner processing it at the Registry.
Our office will release the septic installation permit once we have a copy of the completed restriction. Let me know if
you run into any glitches.

Hi Amy,this looks good with standard conditions. There is a bit of a slope on the property so a section of straw wattle
on the southern side of the house should be included. It looks like the stone wall on the north side will stop any runoff.
